The review also found the off-route alert system was giving limited notifications, and there are gaps in the issue tracking system. Although additional factors were identified, they did not alter the overall conclusion.
The January crash involved a double-decker bus operated by the STA hitting a railroad viaduct, which resulted in significant damage and raised concerns about the safety protocols in place. In response, the STA conducted a thorough review to ensure that all aspects of the incident were fully understood and addressed.
The STA has stated that it will continue to implement measures to improve driver training and safety protocols to prevent similar incidents in the future.
